You should check the terms of this Agreement in detail before committing yourselves to its terms. If you have any questions, please discuss them with a member of staff straight away. After signing up for the plan you will have a 7 days option to cancel. Should you decide to cancel all services already taken under the plan will become immediately payable in full. The plan will continue until all outstanding monies are paid in full. After the 7 day period, you will be bound by the terms. To exercise this option you must contact the practice on 0117 3355999 or visit any of the Animal House Vets Surgeries. Cancellation will only deemed to have been undertaken with a confirmation email from Animal House Vets.

  1. The Plan known as the Active Health Club is designed to allow for monthly payments for treatment administered at intervals under the Plan over a twelve month period. In consideration of the payment by the “Pet Owner” signing up to this contract, “The Pet Owner”, of the monthly fee is to be provided services under the Plan as described in practice literature. The Plan is only as described in practice literature and does not replace the need for a comprehensive pet health insurance policy for all treatments/medications/services not listed in the plan. Any additional prices offered for procedures under the Plan will cover the cost of specific procedures only and will exclude any additional cost associated with the investigation and treatment of any underlying disease/condition.
  2. Animal House Vets may increase the monthly fee on an annual basis and will give one month’s notice of such increase. The practice may increase their charges on products and service outside of the Plan at any time. The monthly payments are inclusive of VAT and all other relevant taxes. VAT rates are liable to change and we reserve the right to change the monthly payments to take account of changes in taxation rates.
  3. The monthly fee will be payable by Direct Debit to Eazy Collect or GoCardless (or its assignee or nominee who collects the fee on behalf of the veterinary practice). The Pet Owner will pay the monthly fee until this contract is terminated.
  4. Animal House Vets may amend or terminate this Agreement by giving to the Pet Owner not less than one month’s notice, provided that all examinations paid for in advance under the Plan have been or will be supplied. The monthly fee must be paid until the termination of the Agreement.
  5. The Pet Owner may terminate by giving written notification of your wish delivered in person or by recorded delivery to terminate the agreement not less than 30 days from prior to the anniversary of the Agreement.
  6. In the event of the monthly charge being unpaid one month after it has become due, Animal House Vets may terminate this Agreement by giving notice to the Pet Owner. In that event, the Pet Owner will be liable for all monies then outstanding and due to Animal House Vets. The Pet Owner will not have to refund the discount in the unfortunate circumstances of the pet dying within 12 months of receiving discounted treatment but the Pet Owner will be liable for any other fees due.
  7. It is the Pet Owner’s responsibility to:
  • Attend the veterinary practice for their pet’s mandatory examinations and check-ups as provided by the Plan within 30 days of the due date of examination.
  • Collect medication as provided for under the plan
  • Inform the Veterinary Surgeon of any injury, conditions or other relevant matter affecting the pet’s health generally.
  1. Products included in the plan will only be dispensed on a three monthly basis. At the point of sign up only 1 month of product will be supplied.
  2. This contract is exclusive to the Pet Owner and is non-transferable between pet owners. It does not cover the services of any other veterinary practice not within the Animal House Vets group of surgeries. Any discounts offered as part of the Plan apply only to those pets on the plan.
  3. All new applications for enrolment will be effective on the earliest available collection date for a direct debit and thereafter on or around the same date.
  4. Any dispute arising under this Agreement will be submitted to arbitration under the Arbitration Act 1996 at the joint cost of both parties. All Notices served under the provisions of this Agreement must be in writing and sent to the last known address of the Pet Owner by recorded delivery post. This agreement is subject to the laws of England and Wales and the exclusive jurisdiction of the Courts of England and Wales.
